California Doula Network Application Requirements

A complete application makes it easier to review your practice and identify the next steps. Use this checklist to prepare, then submit requested information through the secure onboarding process.

Your professional information

Prepare your legal name, professional display name, contact information, and the cities and counties you actually serve. Include the languages you offer, birth and postpartum services, current availability, and any virtual support.

For your public profile, write a clear introduction describing your approach and experience. Use a professional photo you have permission to publish. Keep private identity, tax, and insurance documents out of your public biography.

Training and coverage documents

Have your doula certification or training records, current CPR and first-aid documentation, and professional liability insurance ready for review. The team will explain network requirements and any payer-specific qualifications that apply.

If a document is missing, expired, or under review, describe its actual status. Do not list a qualification or insurance approval you have not received. Document submission begins review; it does not guarantee acceptance.

Provider and billing information

Prepare your individual NPI and any Medi-Cal enrollment approval and applicable health-plan participation records. If you have not completed a step, tell the team so your next steps can be reviewed. Keep approval letters and effective dates available.

Tax information, signed agreements, and payout setup belong in the approved onboarding system. Do not send identity documents, tax numbers, or client medical details through a general website inquiry.

Before accepting an insured client

Confirm your approval for the applicable member plan and requested service, review the written fee and payment terms, and learn the visit-documentation process. Registry membership and payer participation are separate.
