Why Doula Claims Get Rejected—and What Happens Next

Learn how to respond to billing questions without guessing, duplicating submissions, or changing the facts of a visit.

Begin with the actual message

A visit can need corrections before a claim is sent. A payer can also reject a submission or deny a claim after review. These are different stages, so begin by finding the actual status and the explanation attached to the record.

Do not assume every delayed payment means an insurance denial. Ask which step needs attention and who is responsible for resolving it.

Check the information, not just the code

Possible review issues include incomplete documentation, inconsistent provider information, eligibility questions, dates outside an approved arrangement, or a service already submitted elsewhere. The specific record and payer response should guide the correction.

Changing a code without understanding the reason can create another problem. The claim must describe the care actually delivered and meet the applicable billing requirements.

Respond through the approved process

Use the visit or claim identifier, relevant service date, and the requested supporting information. Provide accurate corrections and retain the original history. Keep sensitive details inside the secure account rather than sending them in a public website inquiry.

Avoid creating a second visit to replace a record that is under review. Ask whether the existing record should be corrected and whether the billing team must take the next action.

Understand the outcome and payment status

Correction or resubmission is not a guarantee of reimbursement. Ask whether a payer review, appeal, or other permitted step is appropriate. The team should distinguish the insurance status from your agency payout status.

Under the standard Doulas of California schedule, corrections do not restart the original 45-day submission clock; unresolved payment requirements can still cause a hold. Read the fee and payment explanation and billing service overview.
